Carriage Hill Assisted Living NH offers the following services and amenities as part of its all-inclusive pricing

Accommodations: A private bedroom and bathroom with a walk-in shower, as well as access to all common rooms within the facility including two dining rooms (main and private), four sitting rooms (living room, library, game room and sunroom), laundry room, and terrace, as well as outdoor open spaces designated for resident use for walking, gardening, and relaxing.

Personal Care: On-site staff (typically one staff member for every six residents during day shifts) to assist with all Activities of Daily Living including eating, bathing, grooming and dressing, mobility, toileting, incontinence care and medication management.

Meals and Snacks: Three nutritionally well-balanced meals per day and two snacks per day. Modified diets will be available if ordered by your physician or as otherwise required.

Grooming and Hygiene Supplies: Basic supplies necessary for residents to maintain grooming and personal hygiene including such items as soap, shampoo, toothpaste and shaving cream.

Activities: Daily planned activities, both on and off-site, opportunities for community participation and services designed to meet your physical, social and spiritual needs. Our proximity to the University of New Hampshire, in particular, provides numerous opportunities for intellectual, artistic, musical, sporting and other activities and guest lectures.

Housekeeping: Daily housekeeping services of your room, including your bathroom, as well as all common areas. Housekeeping includes vacuuming, general dusting and emptying of trash.

Laundry Service: Twice weekly laundering of personal washable clothing and bedding. Residents may perform their own laundry if preferred or if needed between scheduled laundry service days at no additional charge.

Phone, Television and Internet: Access to cable television and wireless Internet in all common areas.  Basic cable in all bedrooms.  Phone and wireless Internet-wired bedrooms.

Supervision: On-site staff to provide monitoring and personal care on a 24-hour basis.

Case Management: On-site staff to provide case management services including identification and assessment of your needs and interests, information and referral, and coordination with available resources to best address your identified needs and interests.

Development of Individualized Nursing Care Plan (If Applicable): Registered Nurses on-site to coordinate/develop an individualized nursing care plan if medically appropriate. This plan will be reviewed and modified as appropriate on a monthly basis.

Coordination of Medical & Dental Appointments: Assistance in arranging medical and dental appointments, including assistance in arranging transportation to and from such appointments, and reminding residents of the appointments.
